Information For Incoming & Returning Athletes

ALL STUDENT ATHLETES (New and Returning):

All New student-athletes who will be coming to the CSU Pueblo will be required to provide a physical signed by an MD (doctor), DO, PA (physician's assistant) or NP (nurse practitioner). Physicals signed by other medical professionals (i.e. Chiropractors- DC, nurses- RN, LN) will NOT be accepted. Click Here to access the CSU Pueblo Athletics Physical Form.

All pre-participation paperwork, including a physical for new student-athletes, must be submitted online through the Athletic Trainer System (ATS). All student-athletes should receive an email containing their username and password to begin the paperwork process. Once in ATS, each student-athlete will personalize their profile. Below is a link of detailed instructions on how to complete all required paperwork prior to arriving on campus.

ATS User Instructions

ATS will be an athlete's primary way to contact an athletic trainer. Through this system an athlete can schedule a rehab appointment, update medical paperwork or submit new information. The athletic training staff will use this website to send information as well to student athletes. It is imperative that your contact information as well as emergency contact information is up to date including addresses, phone numbers and emails.

If you did not receive an email with password and user ID information, or have any questions, please contact director of sports medicine Devin Hart (devin.hart@csupueblo.edu).

A CSU-Pueblo Sports Medicine Physician and/or Certified Athletic Trainer's review of the student-athlete's medical record will constitute the approval for athletic participation clearance. Where the student-athlete's health history warrants, the individual may be required to be further evaluated by a physician from the CSU-Pueblo Sports Medicine Center or the appropriate specialist before medical clearance is granted.

***ATHLETES ATTENDING SUMMER CONDITIONING/LIFTING SESSIONS MUST HAVE A PHYSICAL,INSURANCE CARD, AND SICKLE CELL WAIVER SUBMITTED TO ATHLETIC TRAINING STAFF PRIOR TO CLEARANCE FOR PARTICIPATION

TRYOUTS: 

Athletes who will be coming to the CSU-Pueblo campus to try out for an intercollegiate team will be required to provide a physical signed by an MD (doctor), DO, PA (physician's assistant) or NP (nurse practitioner). Physicals signed by other medical professionals (i.e.chiropractors- DC, nurses- RN, LN) will NOT be accepted. Athletes will also be required to sign a sickle cell waiver form.

WALK-ONS:

Athletes who will be walking onto an intercollegiate team at CSU Pueblo will be required to provide a physical signed by an MD (doctor), DO, PA (physician's assistant) or NP (nurse practitioner) on the school's physical form. Physicals signed by other medical professionals (i.e.chiropractors- DC, nurses- RN, LN) will NOT be accepted.
